  • A medication misadventure happens when a patient take either too much or not enough of their prescribed medication. Senior citizens are especially vulnerable because they take so much medication and usually have multiple chronic conditions. Many falls are actually caused by a medication misadventure.

Our Care Team will work with you to reduce the risk of a problem happening to you or your loved one by thoroughly reviewing all your medications for interactions as well as the suitability for your unique genetic make-up.

  • Many patients have multiple doctors prescribing them medications. Unless the patient's chart is kept 100% accurate  at each office, the chance of drug interactions or adverse events increases with every doctor you see. In reality, far too often patients are prescribed medications that duplicate therapy or interact negatively with existing therapy and the only way the patient finds out is when they end up in the emergency room.

Our care team works with you proactively to coordinate care with all of your physicians and to keep you from having preventable adverse reactions.

  • Advances in Pharmacogenomic testing and analysis have now proven that no two patients are identical. Your unique genetic make-up not only makes you more or less susceptible to selected chronic disease but it also determines which drugs are appropriate for you. Research has shown that the drug that works best for you may not work for your spouse. The drug that is safe for your spouse may put you in the hospital. Dosages may need to be adjusted either up or down depending upon your genetic make-up.

Our Care Team will assess your need for a once in your lifetime genomic test and then serve as your genetic consultant.  You should not begin taking any dangerous drug without knowing if the drug is safe and dosed correctly.

Here are a few answers to our most common questions

  • Who will be providing the care for me?

Our clinical pharmacist will work with you to provide you with everything you need. In addition to our pharmacist who will work directly with you, we have a support team of staff and student pharmacists who assist.

  • Can Your Pharmacist Prescribe Medications for me?

State laws regarding what a pharmacist can and can't do vary greatly. But in general, we do not prescribe medications nor does our team dispense any medications. Our aim is to support you in understanding how towork with your physician(s) to get the care you want and deserve.

  • What happens if I sign up for your service?

You will make an appointment with a member of our team who will collect all the information that we need to determine how to deliver the best care possible for you. Once we have agreed on a care plan,  we will develop the details of your plan and schedule a follow-up appointment to go over the results.

  • Can I purchase this service for someone else?

Absolutely!  Many seniors need our services but they do not have the funds or the inclination to order the service for themselves. With our care coordination ability, adult children who are taking care of an aging loved one frequently order this service as a way of monitoring and verifying that their parents are getting the best care possible.

  • What is a Pharmacogenomic test?

The test itself  is a simple cheek swab with a cotton tipped probe. Once the lab analyzes the DNA in the swab we will have a complete record of your Genetic Code. Within this code we can tell which drugs need to have their doses lowered, which need to have the doses increased and which drugs either will not work for you, or pose a significant risk to your health. The only way you can truly know whether the drug your doctor wants to use for you is safe is to compare it to your genetic code.

  • How do you communicate with Patients who enroll?

We use telephone calls,two-way video calls, emails and texts to communicate in a private and secure way. Our Virtual Care Portal connects to computers and smart phones and allows us to invite approved caregivers to the call with the patient.

  • Do I need my Doctor's Permission to work with you?

No you do not. But we work very hard not to interfere with your doctor's care plan for you. We understand that your physician is in charge of your care. We give you the information you need to help prevent adverse reactions, improve outcomes and ask the right questions of your doctor. If we recommend a change to your clinical care, we will give you the documentation to request that change with your physician. But we will never change anything without your doctor's permission.

  • Won't my Doctor be Reluctant to work with you?

Our pharmacy team works directly with several doctor's offices and those physicians see us as a valuable member of their care team. There are so many new drugs and your doctor just doesn't have the time to perform the in-depth analysis that we perform. Most Doctors greatly appreciate the help we give them that helps them provide you with better care.

  • What makes you different from other healthcare profesionals?

Pharmacists are the drug experts in the healthcare system. We do much more than traditional dispensing. Our drug store brethren are too busy filling prescriptions to provide the detailed analysis it requires to provide our level of clinical care.

Our clinical brethren such as Nurses, Physician Assistants, and Doctors are too busy dealing with acute issues to thoroughly analyse the impact of all your disease states and all your medications as well as your unique genetic profile.

The reality is that most patients don't understand how often patients are hospitalized or worse because of a drug misadventure.
